What are the documents required to be a designated workplace doctor?

I want to apply as a designated workplace doctor. I am of legal age and registered with the Singapore Medical Council with a practicing certificate. I want to know the documents that i have to submit when i apply for a registration for be a designated workplace doctor. I also want to know the steps in the application process.

    You need to login and upload the supporting documents to apply. You need to upload a PDR of your, basic medical qualification that complies with the requirements of the Medical Registration Act (Cap 174), back to back bopy of your NRIC or work pass, a valid Medical Registration Certificate, a valid Medical Registration Certificate, and an approved recent postgraduate occupational health qualification.
