Who can share with me the DBS Woman’s Card Supplementary Card Application checklist?

I want to get a supplementary DBS Woman Card for my first daughter. Can someone provide me with details of the Supplementary card application Checklist?

    In order for your daughter to qualify for a supplementary card, she must be at least 18 years old. You will also need to provide a few documents like a copy of your NRIC (Front and Back) for Singaporeans/PR, a copy of your Telecommunication or Utility bill. The annual fees for each supplementary card I think is about $80 but of course she can get to have a fee waiver for the first year.
